Submit this form when a member completes both his Initiatory and DeMolay Degree proficiencies. Also use this form to inform the Nor-Cal office of which members of your Chapter have completed their proficiencies in the past, before the institution of this form. The information on this form will be used to determine eligibility for Jurisdiction/Division elections, activities, and athletic and ritual competitions.
